หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Health Plaza Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

1. This service is reserved for those who present an E-Coupon only.

2. This E-Coupon cannot be exchanged for cash.

3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4. These packages are for outpatients who are physically dependent. 

5. These packages exclude doctor fees in case additional specialists are required.

6. This price only applies to specified test. Doctor fee for interpretation is included. 

7. The price excludes additional tests, investigations or medical consultation in case of positive findings. 

8. This test is intended to screen for individual’s risk of cancers. It is not advised in pregnant women and cancer patients. 

9. These packages exclude treatment fees or medications prescribed by the doctors, if any. 

10. Further information or to make an appointment, please contact the Health Design Center at 02-3103284, 02-7551591 (operating hours: 07:00 - 15:00) or the Contact Center at 1719.

11. Services can be received at the Health Design Center, 5th Floor, Building R, Bangkok Hospital (Soi Soonvijai).

12. The price is applicable to both Thai nationals and expatriates.

 

Note:

1. In case customers request cancellation of the order after the Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.

1. After successful payment, you will receive an SMS with an e-coupon.
2. Please schedule an appointment at least 3 days in advance before receiving the service by calling the Contact Center at 1719 or add line @bangkokhospital
3. Before the service date, please go to "My Orders" -> "To Receive" and click the "Check and Accept" button.
4. Present the e-coupon to the staff on the day of the service.
