เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น
2. ในกรณีที่ไม่นำคูปองมาแสดง ท่านต้องชำระค่าใช้จ่ายตามราคาชุดตรวจ
3. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
4. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
6. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญ
7. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง
8. กรุณาทำนัดล่วงหน้าก่อนเข้ารับบริการอย่างน้อย 3 วัน
9. แพ็กเกจนี้เป็นการพบแพทย์เฉพาะทางอย่างต่อเนื่อง 3 เดือน เพื่อประเมินผลและวางแผนสุขภาพเฉพาะรายบุคคล
10. ไม่รวมค่าจัดส่งอาหารของอิ่ม by BDMS โดยคิดค่าจัดส่งตามระยะทางที่รพ.กำหนดไว้ (สอบถามค่าบริการจัดส่งก่อนสั่งซื้อได้ที่ Line@immbybdms)
11. สอบถามรายละเอียดเพิ่มเติมหรือนัดหมายเพื่อรับบริการได้ที่ Integrated Care Center โทร 02-310-7083 ทุกวัน 08.00 - 16.00 น. หรือ Contact Center โทร. 1719
12. สามารถรับบริการได้ที่ Integrated Care Center ชั้น 2 รพ.กรุงเทพอินเตอร์เนชั่นแนล เวลาทำการ จันทร์-อาทิตย์ 08.00 – 16.00 น. โรงพยาบาลกรุงเทพ สำนักงานใหญ่ (ซ.ศูนย์วิจัย)
13. ราคานี้สำหรับคนไทยและ Expat เท่านั้น
หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Health Plaza Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด
⏬️ Service Terms and Conditions ⏬️
1. Service is reserved for customers who present the coupon upon arrival.
2. In case the coupon is not presented, the customer must pay the examination fee as stated.
3. The coupon cannot be exchanged for cash.
4. The coupon is valid only for the specified examination and cannot be altered.
5. This package is for outpatients who are physically dependent.
6. This package excludes doctor fees in case additional specialists are required.
7. This package does not cover additional treatment or medication prescribed by the doctor.
8. Please make an appointment at least 3 days in advance.
9. This package includes continuous specialist consultations for 3 months to evaluate and plan individualized health care.
10. The delivery fee for meals by BDMS is not included and will be charged according to the distance set by the hospital. If you have any questions about the delivery fee, please contact Line @immbybdms.
11.For further information or to make an appointment, please contact Integrated Care Center at 02-310-7083,
(operating hours: 08:00-16:00) or the Contact Center at 1719.
12.Services can be received at Integrated Care Center, 2nd Floor, Building BIH, Bangkok Hospital (Soi Soonvijai), every day from 8:00 to 16:00.
13. The price is applicable to both Thai nationals and expatriates.
Note:
1. In case customers request cancellation of the order after Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
