3. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. ชุดตรวจสุขภาพ ไม่รวมค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี

6. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

7. กรุณานัดหมายล่วงหน้า อย่างน้อย1 วัน และ ไม่ต้องงดน้ำและอาหาร ก่อนเข้ารับบริการ

10.ราคาแพ็กเกจนี้สำหรับคนไทยและ Expat เท่านั้น

หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Health Plaza Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1. Service is reserved exclusively for customers who present the E-Coupon.

2. This E-Coupon cannot be exchanged for cash.

3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4. These packages are for outpatients who are physically dependent. 

5. These packages exclude doctor fees in case additional specialists are required.

6. These packages exclude treatment fees or medications prescribed by the doctors, if any. 

7. Please make an appointment 3 days in advance and no need to refrain from food and drink. For further information or to make an appointment, please contact the ER & Specialty Night Clinic at 02 310 3003, 02 755

1003 (operating hours: 17:00-23:00) or the Contact Center at 1719.

8. Services can be received at the  ER & Specialty Night Clinic, 1st Floor,  Bangkok Hospital (Soi Soonvijai), every day from 17:00 to 23:00.

9. This price is for Thai citizens and expatriates only.

Note:

1. In case customers request cancellation of the order after Health Plaza Guarantee period without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.

1. After successful payment, you will receive an SMS with an e-coupon.
2. Please schedule an appointment at least 3 days in advance before receiving the service by calling the Contact Center at 1719 or add line @bangkokhospital
3. Before the service date, please go to "My Orders" -> "To Receive" and click the "Check and Accept" button.
4. Present the e-coupon to the staff on the day of the service.
